T he clinical images and the questions that accompany this book ’ s images are pre-
sented in no particular order. They are presented in the same way that patients present

to health - care teams taking care of them in the hospital — randomly. During rounds,
a hospitalist moves from one patient with a unique set of problems and illnesses to
the next patient with a completely different set of unique issues and illnesses. One
patient may have a clinical fi nding easily diagnosed and treated while another may
have a complex fi nding that baffl es the hospitalist and the rest of the team for days
or even weeks. Learning in medicine is mostly random. I believe that we learn best
when fed information in small bite - size pieces that we can refl ect on and absorb and
when these pieces of learning are directly tied to the patients we or our colleagues
are caring for. I have tried to adhere to this principle in creating this book. The
questions are meant to challenge the reader and the answers are intended to teach
or reinforce a few small pieces of related information. I hope that you, the reader,
learn a few things from this book and, most important, I hope you are able to take
better care of your patients somewhere down the road because that which you
learned was helpful.
