The much-delayed onset of concern about diabetes in hospital patients in the UK dates
to 2009–10, when the now disbanded National Patient Safety Agency perceptively
alerted us to poor practice, especially in insulin treatment, that had led to episodes of
significant patient harm. Various insulin training modules appeared shortly after to in-
crease awareness of hospital staff to diabetes management, and I developed one for our
own hospital’s training schedule. In the end, though, I think Information Governance (or
it may have been Slips, Trips and Falls) was thought to be a more pressing must-do topic
for gloomy ‘stat and mand’ training than safe insulin administration, another perverse
consequence of authoritarian centralisation, especially when harm resulting from insulin
misadministration is, rightly, regarded as a ‘never’ event. As I write this preface, however,
I understand the Care Quality Commission is planning to ensure that all hospitals have
an insulin safety programme in place when they pay a visit.
